AFLAC, the covered entity (CE), reported that a technical error allowed the protected health information (PHI) of 1,601 individuals to be viewable on its online web portal. The PHI involved included names, birthdates, addresses, phone numbers, health insurance information, clinical information, and treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards and retrained its staff. OCR obtained assurances that the CE implemented the corrective actions noted.
