Aetna ACE, the covered entity (CE), reported that an employee of its business associate (BA) mailed the protected health information (PHI) of 1,317 individuals to the wrong recipients. The PHI involved included names, addresses, and medications. The CE notified HHS and the affected individuals. In response to the breach, the BA implemented additional administrative and technical safeguards.
