The PHI involved included names, addresses, phone numbers, dates of birth, Social Security numbers, health insurance information, diagnoses, medication information, lab results, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. The CE offered affected individuals complimentary credit monitoring services. Following the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
