The covered entity (CE), Adventist HealthCare, reported that its business associate (BA) mailed letters containing the protected health information (PHI) of 1,300 individuals. The letters were subsequently lost during the mailing process. The PHI involved included demographic and clinical information. The CE notified HHS, the affected individuals and the media. In response to the breach, the CE and BA implemented additional administrative safeguards to better protect PHI.
