Adventist Health Simi Valley

The covered entity (CE), Adventist Health Simi Valley, reported that an employee was the victim of an email phishing attack that compromised the protected health information (PHI) of 54,708 individuals. The PHI involved included names, addresses, dates of birth, health insurance information, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE provided complimentary credit monitoring services and implemented additional technical safeguards, revised its policies and procedures, and retrained its employees on email security. OCR provided the CE with technical assistance.
