The covered entity (CE), Advanced Medical Management reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 319,485 individuals. The PHI involved names, dates of birth, addresses, Social Security and drivers’ license numbers, claims information, diagnoses, lab results, medications, and other treatment information. In response to the breach, the CE implemented new technical safeguards.
