Advanced Fertility Center of Chicago

The covered entity (CE), Advanced Fertility Center of Chicago, reported that an employee was the victim of an email phishing scheme that affected the protected health information (PHI) of 19,000 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, diagnoses/conditions, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. In addition, the CE retrained its workforce members on the proper methods of identifying fraudulent email communications.
