The business associate (BA), Academic HealthPlans, reported that several employees were victims of an email phishing attack that compromised the protected health information (PHI) of 79,263 individuals. The PHI involved included names, dates of birth, Social Security numbers, health insurance and claims information, diagnoses, and other treatment information. In response to the breach, the BA implemented additional technical safeguards and retrained employees on email security.
